  • PRIVIT UPDATE - June 12

    In response to the COVID-19 pandemic, the IHSAA has required an IHSAA Health History Update Questionnaire and Consent & Release for 2020-21 year. In order to participate in  Practices and  Contests at Avon Middle School North during  the  2020-21  school  year on  a  school-sponsored  team, a student who  had  a  prior  pre-participation  physical  examination completed and  such  examination  was completed more than 90 days prior to the first day of official practice for the student’s sport, may, in lieu of having a 2020-21 Pre-Participation Physical Examination form completed, provide this Health History Update Questionnaire, completed and signed by the student’s  parent or  guardian, or  by  the  emancipated  student. Provided, should  any  question on  this Questionnaire be  answered in the affirmative (‘Yes’), then  the student  must have  a  2020-21  Pre-Participation  Physical Examination form completed. This form will be completed in Privit. All student-athletes must fill out this form prior to participating in any school workout. All clearance status’ have been reset in Privit.

  • Attention all Parents and Prospective Student-Athletes:

    I know there are questions about athletics and physicals floating around. I want to try and answer some of these. Please know this is an evolving situation and things may change. I am hoping to continue to keep lines of communication open for you all. Also, please note that as of right now, doctors are only seeing patients dealing with a wellness issue. I have personally reached out to my son’s doctor to find out if they are currently performing wellness check-ups and/or physicals. I was informed that doctors are hoping that over the summer they will be able to start making appointments for athletic physicals. I would suggest you also reach out to your medical provider, when/if you have questions about scheduling appointments. I share the following information for when those types of appointments can occur.

    Any student wishing to participate in athletics for the 2020/21 school year must have a physical on file through our PRIVIT system. This is a requirement for any student-athlete wishing to participate in an open gym, practice, tryout or game.

    Upon receiving guidance from the IHSAA recently, students who will be in 8th grade next year and have a physical and account on file from the 19/20 school year will be allowed to use that same physical for 20/21. Any incoming 7th grader, new enrollee or student currently at North without a physical wishing to participate in sports will need to still obtain a physical and upload that into PRIVIT while also signing necessary forms.

    Physicals can be obtained through a doctor’s office, wellness clinics, minute clinics, etc. Please see the above about a possible timeline those types of appointments may begin to take place.

    Current students with an account may be asked to re-sign necessary forms on the PRIVIT site.

    As we navigate this situation, more information will be put on our webpage. Directions for PRIVIT are currently posted on the main athletics page. There will be a period of time when the site is down for a rollover to occur. We will do our very best to continue to give updated information as we receive it.

  • Requirements for Athletic Participation - PRIVIT

    For the 2019-20 school year ACSC is continuing to use a completely paperless clearance process.  The process includes a paperless IHSAA Pre-Participation Physical. A parent/guardian and student-athlete will be required to create an account and manage the necessary documents using the Privit Portal. This process will provide a higher level of security and efficiency than the previous process.  This process will be for everyone including those coming to ACSC Physical Night and those students who are going another place to get a sports physical.     Please note: you need to select a team when you create your PRIVIT account. You can select all teams that your student is interested in joining.

  • Athletic Fees are collected for each sport after rosters are finalized. Athletic Fees include the following:

    Athletic Fee: $25 (this is one-time per school year)

    Program Support Fee: $50 (per sport)

    Practice Pack: varies per team

    Coaches of each sport will collect their fees once rosters are finalized. Fees are due before the first scheduled contest for each sport unless other arrangements have been made with the Athletic Office.

  • Travel Release Consent Form
    If your student athlete will not be riding the school provided transportation, please sign the Contest Travel Release for each event. This form can be found HERE or on the Athletic Forms page. If you have questions, please contact Mr. Keller, or your child's coach.

    Admission Prices: All home, non-tournament contests hosted by Avon Middle School North have the following admission rate:

    Students and Adults:  $4.00
    We will offer 10-game athletic passes for $30 each. Passes can be purchased in the main office or at the gate, and are good for regular-season contests. They are NOT accepted at any tournaments, including County, Conference and Invitationals.
