  • In an effort to help prepare for the 2020-2021 school year, ACSC has partnered with Hendricks Regional Health to provide quality, low cost pre-participation physical exams. This partnership allows each school to keep a portion of the fees charged from each exam completed. Students MUST have a physical on file dated after April 1st, 2020 before being allowed to participate in summer activities. 

     

    IMPORTANT FACTS 

    Who — All 2020-2021 middle school & high school students  

    When — Monday, April 20th at Avon High School starting at 5:45pm 

    Cost — $25.00 (paid at the time an arrival time is scheduled.)  

    Physical Form — Please login using your PRIVIT login from last year when entering information for 2020-2021. IHSAA has changed the physical for the 2020-2021 school year. 

     

    To schedule an appointment/arrival time:  

    Check AHS/AMMS/AMSN Athletic website for link to schedule “arrival time” starting the week before Spring Break (March 23rd, 2020).  

     

    Plan to arrive no earlier than 10 minutes before the scheduled appointment time. The complete physical exam will take approximately one hour.

Ms. Angie Miller

Angie Miller earned a bachelor’s degree at Ball State University and a master’s degree at Ohio University. After a short stint at Baxter Physical Therapy for one year, Angie moved back to Indiana in 1991 to be a teacher and athletic trainer at Avon High School where she has worked for over 20 years. Angie’s involvement with high school students has extended beyond working as an athletic trainer. Since 1991, she has served as the Fellowship for Christian Athletes Advisor.She has been an instructor and counselor for numerous Indiana Pacer Student Athletic Trainer workshops. Angie has been involved with the Indiana Athletic Trainers Association having served as the IATA High School Representative (1994 – 96) and Treasurer (1996 – 2002).

Mr. Dillon Spencer

Dillon is currently an athletic trainer at Avon High School. He joined the Hendricks Regional Health sports medicine team in the summer of 2015 where he began working at Avon Middle School South. Dillon was moved to a full time position at Avon High school in the fall of 2017. Dillon graduate of Franklin College where he received a Bachelor’s degree in Athletic Training and a minor in fitness. He is a member of the Indiana Athletic Trainers Association and the National Athletic Training Association.

Ms. Miranda Lofgren

Miranda is currently an athletic trainer at Avon High School. She joined the Hendricks Regional Health Sports Medicine Team in July 2016 to work as the athletic trainer at Avon Middle School North.  She completed her master degree at Indiana University with MS in Athletic Training in 2016. She completed her undergraduate degree at Millikin University with a B.S. degree in Athletic Training and a minor in Nutrition. While at Indiana University, Miranda worked as a graduate assistant at Owen Valley High School/Middle School and with Indiana University Rowing. While at Millikin University, she worked with the football team, softball team and men’s and women’s track team. She completed clinical rotations in various healthcare settings and at a local high school. Miranda is a member of Indiana Athletic Trainers Association and the National Athletic Trainers Association.

Mikayla Patterson

Mikayla joined the Hendricks Regional Health Sports Medicine team in July 2019 to work as the athletic trainer at Avon Middle School South. She completed her undergraduate degree at Manchester University with a B.S. in Athletic Training. While at Manchester University Mikayla worked with the football team and men's soccer team. Mikayla also worked with Whitko high schools Men's and Women's basketball team. She was also involved in the Manchester University Athletic Training Club and excercience club. While at manchester she received the Dwayne Dixon Scholarship from the Indiana Athletic Trainers Association. She is also an active member of the Indiana Athletic Trainers Association (IATA) and the National Athletic Trainers Association (NATA).

Ms. Shelby Hawkins

Shelby joined the Hendricks Regional Health Sports Medicine Team in July 2019 to be the athletic trainer at Avon Middle School North. She completed her undergraduate degree at The University of Indianapolis with a B.S. degree in Athletic Training. While at The University of Indianapolis, Amanda worked with the girls lacrosse team, track and field, baseball, men’s and women’s basketball, and wrestling. She completed clinical rotations at high school settings, office settings, Divison I and Divison II colleges. Shelby is a member of Indiana Athletic Trainers Association and the National Athletic Trainers Association.
