    Application for Leave of Absence

    If you wish to request a leave of absence that may qualify under the Family Medical Leave Act (FMLA), you should do the following:

    New Leave of Absence Procedure:

    Avon Schools has partnered with an outside firm, Sedgwick, to handle all Leave of Absence/FMLA paperwork.  Effective August 1, 2019, Sedgwick will be taking over the process for all employees currently on a Leave of Absence/FMLA and for any employees needing to apply for a new Leave of Absence/FMLA.  With the increase in the number of employees working for Avon Schools and the federally regulated requirements that are needed with regard to Leaves of Absences/FMLA's, it was necessary to partner with Sedgwick to ensure all documentation is compliant and meets the standards required by the federal government.

    Anytime an employee is absent for more than three (3) consecutive days or is aware that due to a medical reason he/she will need to be absent more than three (3) consecutive days, that employee will need to contact Sedgwick to complete FMLA/LOA paperwork.  Please refer to the three (3) Sedgwick documents to left for Sedgwick contact information and the details regarding requesting a Leave of Absence and/or FMLA.  When communicating with Sedgwick regarding a Leave of Absence/FMLA, if the Sedgwick representative is unable to answer your questions, please feel free to reach out to the Avon Schools Benefit Coordinator, Joyce Genneken, at 317-544-6048 or jegenneken@avon-schools.org.
