    • In response to the COVID-19 pandemic, the IHSAA has required an IHSAA Health History Update Questionnaire and Consent & Release for 2020-21 year. In order to participate in  Practices and  Contests at Avon Middle School South during  the  2020-21  school  year on  a  school-sponsored  team, a student who  had  a  prior  pre-participation  physical  examination completed and  such  examination  was completed more than 90 days prior to the first day of official practice for the student’s sport, may, in lieu of having a 2020-21 Pre-Participation Physical Examination form completed, provide this Health History Update Questionnaire, completed and signed by the student’s  parent or  guardian, or  by  the  emancipated  student. Provided, should  any  question on  this Questionnaire be  answered in the affirmative (‘Yes’), then  the student  must have  a  2020-21  Pre-Participation  Physical Examination form completed. This form will be completed in Privit. All student-athletes must fill out this form prior to participating in any school workout. All clearance status’ have been reset in Privit.

       
    • Attention all Parents and Prospective Student-Athletes:

      ANY student moving from 7th grade to 8th grade or 8th to 9th grade that had a physcial for the 2019-20 school year will need to complete the IHSAA health history (supplemental questionaire) ONLY.  ANY 6th grader coming to 7th grade or any student that did not have an IHSAA physical for the 2019-20 school year MUST HAVE AN IHSAA PHYSICAL FORM COMPLETED BY A LICENSED PHYSICIAN before they will be eligible to attend a tryout or practice for Avon South. 

      Any student wishing to participate in athletics for the 2020/21 school year must have a physical on file through our PRIVIT system. This is a requirement for any student-athlete wishing to participate in an open gym, practice, tryout or game.

      Upon receiving guidance from the IHSAA recently, students who will be in 8th grade next year and have a physical and account on file from the 19/20 school year will be allowed to use that same physical for 20/21. Any incoming 7th grader, new enrollee or student currently at South without a physical wishing to participate in sports will need to still obtain a physical and upload that into PRIVIT while also signing necessary forms.

      Physicals can be obtained through a doctor’s office, wellness clinics, minute clinics, etc. Please see the above about a possible timeline those types of appointments may begin to take place.

      Current students with an account may be asked to re-sign necessary forms on the PRIVIT site.

      As we navigate this situation, more information will be put on our webpage. Directions for PRIVIT are currently posted on the main athletics page. There will be a period of time when the site is down for a rollover to occur. We will do our very best to continue to give updated information as we receive it.

     STUDENTS MUST HAVE A PRIVIT ACCOUNT BEFORE ATTENDING TRYOUTS

     

    For the school year ACSC is moving to a completely paperless clearance process.  The new process includes a paperless IHSAA Pre-Participation Physical. A parent/guardian and student-athlete will be required to create an account and manage the necessary documents using the Portal Privit. This process will provide a higher level of security and efficiency than the previous process.  This process will be for everyone including those coming to ACSC Physical Night and those students who are going another place to get a sports physical. 

    YOU ONLY NEED TO UPLOAD PAGE 2 of the PHYSICAL.  THE PAGE THE DOCTOR SIGNS IS MOST IMPORTANT! 

     

    Frequently Asked Questions:

    I can't get my child an IHSAA physical until practice already starts is that OK? No. A student must have their physical and PRIVIT account set up prior to practice.  Our students have to have 10 practices in before the first contest and any delay will have them missing ahletic contests potentially. 

    Is there a bus to bring my child home after practice each day? We do not have an activity bus to bring students home after practice each day. Transportation home must be provided by parents or friends. 

    What are the fees used for? The $50 program support fee will be paid everytime your child makes a team at Avon or participates in a sport.  Those fees are to help off set the costs it takes Avon to have extra curricular activities.  The $25 atheltic fee is paid ONE TIME A YEAR and helps our athletic department buy new uniforms, balls, ribbons etc. to help keep our equipment acceptable. 

    How will the coach communicate with parents on practice times or changes? Parent Square.  All information will be through parent square!

    Athletic Fees:

    The Athletic Fee is $25.00. This is a one-time fee that all athletes pay once they join a team. The Program Support Fee is $50.00 per sport.
     

    **ALL SPORTS REQUIRE THE FOLLOWING FOR PARTICIPATION IN OPEN GYMS, WORKOUTS, AND TRYOUTS:

    **PASSES ARE NOT VALID FOR TOURNAMENTS** AMS is not responsible for lost or stolen cards.  Passes must be shown at the gate and are valid only for AMS athletic events.
