Employee Injury Reporting

The ACSC School Board directs that reasonable efforts be made to ensure a safe working environment for the employees of the Avon Community School Corporation.

Employees are required to report all accidents. Any accident which results in an injury to an employee; however slight, must be reported promptly and in writing to Human Resources. Injured employees will be referred immediately to the school nurse or appropriate personnel for medical attention as needed. 

The Accident Report should be completed by the employee and sent to Human Resources within 24 hours of the injury. The failure of an employee to comply with this mandate may result in disciplinary action.

IF THE INJURY IS A LIFE-THREATENING EMERGENCY, CALL 911.

Non-Life Threatening Injuries - Employees Should Do the Following:

Step 2: Complete the Accident Report within 24 Hours

As of April 2026, the Accident Report Form is now electronic using Microsoft Forms. Click the link to the form to complete all required information. Information submitted goes directly to ACSC Benefits.

Step 3: If Needed, Seek Medical Attention

See a School Nurse

  • The school nurse can treat minor injuries.
  • If further treatment is needed, the nurse can provide an "Authorization for Services" to be seen at one of the HRH Occupational Health facilities.
    • If a nurse is not available to authorize services, please contact the HR-Benefits Team
    • For 2nd and 3rd shift workers, contact your direct Supervisor.

    • Hours: Monday - Friday, 8am - 4:30pm
    • After 4:30pm, patients can be seen at the neighboring Urgent Care Center until 8pm, Monday-Friday or 8:30am - 4:30pm Sat/Sun.

Indiana Worker's Compensation

Worker’s compensation is an accident insurance program paid by the school which may provide you with medical, rehabilitation and income benefits if you are injured on the job. These benefits are provided to help you return to work. 
 
  • Refer to both School Policy and the Worker's Compensation Board of Indiana
  • In Indiana, the employer directs medical care.
    • ACSC has partnered with HRH Occupational Health Centers for work-related medical care. HRH may provide referrals to specialists and further treatment, if needed.
    • The employee must maintain all follow-up appointments and follow all restrictions and treatment instructions given.
  • If you are unable to work for more than seven days, you are entitled to weekly income benefits. School policy requires the use of any available paid time off (sick, personal, vacation) to cover any lost workdays during the initial seven day wait period.
